JOB SUMMARY:Working within the context of a client & family centered health care delivery model, based on a philosophy of self care management and in accordance with the vision and values of the Vancouver Coastal Health Authority, North Shore/Coast Garibaldi Service Delivery Area and in accordance with the College of Registered Nurses of British Columbia (CRNBC) Standards of Practice, plans, delivers and evaluates the care needs of acutely, chronically and terminally ill clients in community settings, initiating referrals to other professionals or agencies where required. Provides education and counseling to clients/families/caregivers. Consults, confers and collaborates with interdisciplinary team members, educational resources and other health care professionals/providers to achieve excellence in health services and education.
QUALIFICATIONS:Education & Experience
Current practicing registration with the College of Registered Nurses of British Columbia. Two years experience as a Community Health Nurse or an equivalent combination of training, education and experience. Valid BC Drivers License. Local area travel requires the use of a personal vehicle.
Knowledge & Abilities
Comprehensive knowledge of community health nursing theory and practice within a client and family centred model of care. Comprehensive knowledge of the College of Registered Nurses’ of British Columbia Standards for Nursing Practice. Comprehensive knowledge of evidence based nursing practice related to home care nursing services for clients with complex home care needs. Comprehensive knowledge of community resources available for client/family support and related health services. Demonstrated ability to practice using the North Shore case management model to case manage/care coordinate client and family health issues based on definition of case management. Comprehensive knowledge of other health care disciplines and their role in patient care. Broad knowledge of research process and methodology. Demonstrated ability to work collaboratively as a member of an interdisciplinary team. Demonstrated ability to communicate orally and in writing. Demonstrated ability to communicate with, and deal effectively with, co-workers, physicians, other health care staff, clients and their families, and outside agencies. Demonstrated ability to adjust to new or unexpected events. Demonstrated ability to plan, organize and prioritize work. Demonstrated ability to deal with and/or guide others in resolution of conflict issues. Demonstrated skill in the use of medical equipment and supplies appropriate to home care nursing. Demonstrated ability to critically read and utilize current health research in evidence based practice. Demonstrated skill in CPR techniques. Demonstrated physical ability to perform the duties of the position. Basic computer literacy. Ability to operate related equipment.
